An arrest warrant was issued Monday for a Johnstown man who allegedly caused a head-on collision in Connellsville March 2, causing serious injuries.

David Michael Allen, 46, of Barron Avenue was reportedly driving under the influence of Tramadol, an opiate, and crashed near Fiesta Insurance Agency at 325 Memorial Boulevard at about 4:45 p.m.

The driver of the second vehicle, Mark E. Dobrick, and Allen and his wife, Ann Marie Allen, all suffered serious and potentially life-threatening injuries, according to the affidavit of probable cause filed in his case. They were each flown by medical helicopters to Pittsburgh hospitals.

David Allen was found to have Tramadol “well above therapeutic levels” in his bloodstream, the affidavit said. He told police at the hospital he did not remember what happened, saying he was talking to his wife and “the next thing he was in a helicopter.”

A witness, Tyler Broadwater, told police Dobrick did not appear to be speeding. He said he saw Allen’s pickup truck cross over the raised median, hitting the second pickup truck, court paperwork indicated.

Police observed several prescription bottles in the vehicle and a nearly empty fifth-gallon of rum. He did not have alcohol in his bloodstream, according to the affidavit.

Police said his license was suspended. Evidence indicated he was driving between 50 and 54 miles per hour in a 40-mile-per-hour zone at the time of the crash.

Allen is charged with two counts each of aggravated assault by vehicle while DUI, aggravated assault by vehicle, accidents involving death or personal injury and one count of driving under the influence.

Charges were filed Monday before Magisterial District Judge Ronald Haggerty Jr.
